Jack Harmon Culbreath II age 34 of Damascus died Wednesday, October 21, 2015 at Cherokee Village. He was born January 15, 1981 at Sulphur Rock, Arkansas to Jack Harmon Culbreath and Bernice Daphene Smith. He was a pipeliner and a member of the Sulphur Rock Baptist Church. He enjoyed hunting, fishing and all sports especially basketball. He was preceded in death by his parents; two brothers, Bobby Culbreath and Perry Culbreath and one sister, Harmony Culbreath. Survivors include one son, Jack Harmon Culbreath III of Grenada; one brother, Marty Smith of Jacksonville; three sisters, April Hensley and husband Terry of Damascus, Tanya Smith and fiancé David Mills of Searcy and Marsha Smith of Batesville; several nieces and nephews and a host of other relatives and friends. Funeral services will be Thursday, October 29, 2015 at 2:00 p.m. at the Qualls Funeral Home Chapel in Cave City with Rev. Brian Schlabach officiating. Arrangements by Qualls Funeral Home of Cave City. Visitation will be Wednesday, October 28, 2015 from 6:00 until 8:00 p.m. at the funeral home. Honorary pallbearers are Mikel Melton, Christopher Shaul, Rusty Pate, Esias Bautista, Aiden Melton, Jesse Hensley, Nick Page and John Riley Pate.
